Discussion

In is sometimes easy to over look recording such discussions, but such purposeful conversations are an important part of the care plan for a young person.

Equally it is important to avoid over recording.

In practice you might want to make a note as the worker to follow up this conversation with Dean at a later stage.

If you were recording using the model for recording in residential care in Write Enough, this discussion would be recorded in Dean’s personal records in the ‘Active File’. These would be placed in Dean’s individual file at a later date.
