Jackie was becoming loud and verbally aggressive in the kitchen. She was asked to leave and then smeared Pizza down the hall wall. Becoming more angry she swept things off the tables in the dinning room then went to back to the hall and threw a picture on the floor.

I tried to calm her down, but as she had been drinking and was getting more angry and out of control, I decided that Tom and I might have to restrain her.

Jackie picked up a vase of flowers from the hall table and threw its contents towards Tom.

I told Jackie that she should stop or we would have to stop her as we did not want her to be hurt.

She immediately smashed a pane of glass in the door with the vase.

I decided then that we had no option but to restrain Jackie to prevent her from hurting herself further.

Discussion

Such incidents require more detailed recording of the facts, including what actions you took and should include the judgements you made. It is important to demonstrate that agency policies were followed in any incident where restraint is used.

In practice the recording would include a report on the actions taken by practitioners to restrain the young person.

If you were recording using the model for recording in residential care in Write Enough, the fact that an incident occurred with Jackie should be briefly recorded in the unit log. This should direct the reader to the ‘Active File’ where a detailed record of the incident would be made in Jackie’s personal records. These would be placed in Jackie’s individual file at a later date. This may be on a separate ‘Incident Record’ form.
